Records Procurement

The right records—complete, authenticated, and delivered securely.

Successful discovery depends on obtaining the correct records from the correct custodian. Lopez & Associates manages the records-procurement process from the initial request through final production, allowing your legal team to focus on the case.

We obtain medical, billing, radiology, pharmacy, employment, insurance, governmental, educational, and other business records through authorizations, subpoenas, and other legally permitted requests.

Based in California, we coordinate records procurement nationwide and, when needed, work with providers and custodians outside the United States.

With decades of experience navigating discovery, healthcare systems, government agencies, and third-party ROI companies, we know that receiving some records is not the same as receiving the complete record.

Our services include:

  • Identifying facilities, custodians, and release-of-information providers
  • Preparing and transmitting properly directed requests
  • Tracking authorization, subpoena, notice, and production requirements
  • Following up on outstanding, incomplete, or misdirected requests
  • Coordinating custodian fees and required declarations
  • Reviewing productions for apparent gaps or missing record categories
  • Securely delivering records in an organized digital format

Deposition Officer Services

Independent handling that protects the integrity of the record.

A deposition officer is more than a copying service or document courier. The deposition officer serves as a neutral third party in the collection, receipt, preservation, reproduction, and delivery of subpoenaed business records.

This independent role helps preserve the integrity of the discovery process by establishing that records were received directly from the responding custodian and handled separately from the parties to the action.

We serve applicant, plaintiff, defense, employer, carrier, claims-administrator, and governmental interests without changing our responsibility: manage the record neutrally, accurately, securely, and in accordance with the applicable discovery process.

Lopez & Associates coordinates:

  • Preparation and issuance of business-record subpoenas
  • Required notices and supporting documents
  • Communication with custodians and their authorized representatives
  • Direct receipt and secure preservation of produced records
  • Custodian declarations and certifications
  • Reproduction and distribution to authorized parties
  • Documentation of nonproduction, objections, or custodian responses
  • Secure electronic access and organized record delivery

When the authenticity, custody, and completeness of the record matter, the deposition officer matters.

Subpoena & Process Serving

Proper preparation. Documented service. Reliable follow-through.

A subpoena is only effective when it is correctly prepared, directed to the appropriate person or entity, and served in accordance with the applicable requirements. Lopez & Associates coordinates subpoena preparation and process serving throughout California and beyond through qualified local resources.

We also coordinate the delivery of medical records and related correspondence to physicians conducting Qualified Medical Evaluations (QMEs), Independent Medical Examinations (IMEs), and other medical-legal evaluations. Materials may include client-provided cover letters, attestation correspondence, exhibits, and supporting records, with service documented upon completion.

Every assignment is tracked from receipt through completion. If an entity has moved, uses a registered agent, delegates records to an ROI company, or disputes responsibility for the requested records, we investigate the issue and report what happened.

Our services include:

  • Business-record and deposition subpoena preparation
  • Identification of the proper custodian, business entity, or agent for service
  • Personal service and substituted service when legally permitted
  • Service on medical providers, employers, insurers, corporations, government agencies, and individuals
  • Witness-fee coordination when required
  • Subpoena proofs and declarations of service
  • Due-diligence attempts and status reporting
  • Rush, same-day, and difficult-location assignments when available
  • Coordination of out-of-area and out-of-state service
  • Delivery of medical records and client-provided correspondence for QME, IME, and other medical-legal appointments
  • Documented service with proof or confirmation of delivery

You receive more than a service attempt—you receive documented accountability throughout the assignment.

Imaging & Copy Services

Turn paper and unstructured files into organized, usable case records.

Legal records are only valuable when your team can locate, review, cite, and produce the information they contain. Lopez & Associates converts paper and electronic documents into secure, organized files designed for practical legal use.

We work with medical records, billing records, claims files, employment records, legal files, exhibits, and other document-intensive productions.

Our goal is not simply to scan pages. It is to preserve the record while making large productions easier to navigate, distribute, and use throughout discovery, evaluation, settlement, hearing, or trial.

Our imaging and reproduction services include:

  • High-volume document scanning
  • Searchable PDF creation and optical character recognition
  • Bates numbering and document labeling
  • Record indexing and logical file organization
  • File conversion, merging, splitting, and quality review
  • Duplicate-set and production copying
  • Secure electronic delivery and authorized file access
  • Printed binders, exhibits, and working copies
  • On-site or coordinated off-site copy assignments
  • Processing of mixed paper and electronic productions

From physical documents to searchable digital files, we make the record work for you.
